Bringing Dignity and Hope through Supportive Community Transition: The Journey Home - A Hospital-Community Agency Wellness Initiative

The Journey Home is a transitional supportive housing program that provides healthcare supports for people living with mental health challenges. The five houses located at 180, 186, 202, 204 and 208 Church Street will become a home and program for 15 people who will be assisted by LOFT Community Services staff after they have been discharged from Humber River Hospital’s inpatient program.
